Risk of Injury
Participating in any sport involves inherent risks of injury. At Mindful Hoops LLC, we have taken reasonable precautions to minimize the risk of significant injuries by providing competent coaching, well-maintained facilities, and proper conditioning. While the chances of sustaining a catastrophic sports injury are extremely remote, it is important to acknowledge that serious injuries can happen to anyone. With this understanding, we require the following waiver and release of liability.
Waiver and Release of Liability
By checking the agreement box when registering, the parent or legal guardian (“the undersigned”) acknowledges and accepts the risks associated with participating in the Mindful Hoops LLC basketball program. The undersigned waives and releases Mindful Hoops LLC, including its coaches and staff, from any liability that may arise during participation in our basketball program. It is understood that the undersigned carries personal medical insurance for their child in case of accident, injury, or illness.
The undersigned gives consent and approval for their child to participate in the Mindful Hoops LLC basketball training programs, and discharges, waives, and releases Mindful Hoops LLC, its coaches, and its staff from any and all liabilities.
Checking the agreement box during registration has the same effect as signing this waiver. For details on other program policies, see our Terms & Conditions.
